Leadership & Corporate Office
Located in downtown Bloomington, Illinois, the Heritage Operations Group corporate office is in the former Roland Department Store building. Renovations on the building were completed in 1989, and parts of it were restored to its original specifications.
The work of employees at the corporate office supports the individual Heritage and Evergreen facilities throughout Illinois. Corporate provides the locations with supplies, materials, and information - whatever is needed to support each facility, the team serving there, and in turn, each and every resident.
Benjamin Hart, President & Chief Executive Officer
Benjamin Hart is the president and chief executive officer of Heritage Operations Group LLC and Green Tree Pharmacy, Inc. He also serves on the board of directors for both organizations. Ben has worked in the health care industry since 1996, all of which have been with Heritage and Green Tree. Before assuming his current role, Ben served as executive vice president of administration for Heritage. He was responsible for the new development and construction of numerous assisted and supportive living facilities, major renovations and capital reinvestments, acquisitions, facilities management, capital purchasing, environmental services, information technology, and marketing.
Ben is a licensed nursing home administrator and served as an administrator for several years at Heritage Health of Bloomington. Ben graduated from the University of Colorado at Boulder with a Bachelor of Science in Accountancy. After completing undergraduate studies, he went on to earn a Master’s of Business Administration from Illinois State University.
Ben serves on the board of directors for Affordable Assisted Living Coalition, and is the past president. He also serves on the boards of the Health Care Council of Illinois, Advocate BroMenn Philanthropy Council, Bloomington-Normal Economic Development Council, Illinois State University Foundation Board, and the Illinois State University College of Business Advisory Council. He also served as co-chair for the OSF Health Care Foundation crystal ball.
Connie Hoselton, Executive Vice President, Operations & Chief Operations Officer
Connie Hoselton is the Executive Vice President of Operations and the Chief Operations Officer for Heritage Operations Group. She has worked in the health care industry since 1988, all of which have been with Heritage. Connie is responsible for skilled nursing facility operations, dietary, marketing, risk management, and human resources components at Heritage.
Connie is certified as a licensed nursing home administrator and a Senior Professional in Human Resources (SPHR) by the Society for Human Resources Management (SHRM).
Connie is a member of the Illinois Nursing Home Administrators Association. She also is a member of the Central Illinois Employers Association and the Society for Human Resource Management, locally the Bloomington Normal Human Resource Council. She currently serves on the Illinois State University Distinguished MBA award panel. Connie is a former board member for the Baby Fold, the Employers Health Care Coalition (ECHI) of McLean County, the Center for Human Services, and the Illinois State University College of Business Advisory Board.
Connie earned a Bachelor of Science degree from Eastern Illinois University and a Master of Business Administration degree from Illinois State University.

David Underwood, Executive Vice President, Finance and Chief Financial Officer
David M. Underwood, CPA, is the Executive Vice President of Finance and Chief Financial Officer for Heritage Operations Group. He is responsible for corporate-wide finance, accounting, budgeting, reimbursement, and reporting. David is also heavily involved in merger and acquisition analysis, project financing, and banking relations.
David has extensive experience in healthcare finance and administration, having served in public accounting, physician office and acute care facility venues.
He is a member of the Healthcare Financial Management Association and the American Institute of Certified Public Accountants. He earned a Bachelor of Arts in Economics from DePauw University and has been a CPA since 1980.

Jenny Clyatt, Senior Vice President of Technology & Chief Information Officer
Jenny Clyatt is the Senior Vice President of Technology & Chief Information Officer for Heritage Operations Group. She is responsible for corporate-wide information technology operations and strategy. Jenny has more than 30 years of healthcare experience, including both Nursing and IT management. Before joining Heritage in February of 2013, she served as Administrative Director of Health Informatics & Technology for Proctor Hospital in Peoria.
Jenny is a member of the College of Healthcare Information Management Executives and Healthcare Information and Management Systems Society. She is a Registered Nurse and graduate of Mennonite College of Nursing in Bloomington.
