Reopening Phases and Locations

February 23, 2021

In accordance with the Illinois Department of Public Health's (IDPH) reopening guidance, we must report to you, in written format, the Centers for Medicaid & Medicare Services (CMS) phase for our community and must meet certain criteria to advance to the next phase. Our community must spend a minimum of 14 days in a given phase and meet requirements set forth by IDPH and CMS. If our community identifies a new-onset COVID-19 case, it must revert immediately to the highest level of mitigation and start over in the phases of reopening.

Information will be updated weekly on our community's website and Facebook page every Wednesday at 4 p.m. Please contact the facility for additional information.

Thank you for your extreme patience and understanding during this time of challenge and changing guidelines.
