Reopening Phases and Locations

In accordance with the Illinois Department of Public Health's (IDPH) reopening guidance, we must report to you, in written format, the Centers for Medicaid & Medicare Services (CMS) phase for our community and must meet certain criteria to advance to the next phase. Our community must spend a minimum of 14 days in a given phase and meet requirements set forth by IDPH and CMS. If our community identifies a new-onset COVID-19 case, it must revert immediately to the highest level of mitigation and start over in the phases of reopening.

Information will be updated weekly on our community's website and Facebook page every Wednesday at 4 p.m. If you do not have access to the Internet or Facebook, please contact our community's Life Enrichment Coordinator for information.

PPHASE 1:

  1. Indoor visitation is prohibited with the exception of compassionate care visitation.
  2. Outdoor visitation may occur in an outdoor space designated by the community. Please contact the community for further information.
  3. Virtual visitation is available to all residents via several video conferencing platforms.
  4. Compassionate care visitation is considered on a case-by-case basis.
  5. Beauticians/Barbers are not permitted.


PHASE 2 (Subject to the approval of the local health department):

  1. Indoor visitation may occur, unsupervised, in common spaces or residents' apartments, with a six-foot separation and cloth face covering or masking by visitors and residents. Contact the community for further information.
  2. Outdoor visitation may occur in an outdoor space designated by the community. Please contact the community for further information.
  3. Virtual visitation will continue to be available to all residents via videoconferencing platforms.
  4. Compassionate care visitation is considered on a case-by-case basis.
  5. Beauticians/Barbers are permitted at the discretion of the community.


PHASE 3 (Subject to the approval of the local health department):

  1. Indoor visitation may occur, unsupervised, in common spaces or residents' apartments, with a six-foot separation and cloth face covering or masking by visitors and residents. Contact the community for further information.
  2. In-room visitation is permissible for bedbound patients. Please consult with the community for further information.
  3. Weather permitting, outdoor visitation is strongly preferable to indoor visitation.
  4. Outdoor visitation, unsupervised, is permitted. See Phase 2.
  5. Virtual visitation is available to all residents via videoconferencing platforms.
  6. Compassionate care visitation is considered on a case-by-case basis.
  7. Beauticians/Barbers are permitted.
