In accordance with the Illinois Department of Public Health's reopening guidance for nursing homes, we must report to you, in written format, the Centers for Medicaid & Medicare Services (CMS) phase of a nursing home and when each nursing home meets certain criteria to advance to the next phase. Each home must spend a minimum of 14 days in a given phase and meet requirements set forth by IDPH and CMS. If the facility identifies a new facility-onset COVID-19 case, the facility must revert immediately to the highest level of mitigation and start over in the phases of reopening.


Phase descriptions are below with a listing of which phase our buildings are in. Please note this list will be updated every Wednesday by 4 pm with any changes.

  1. Indoor visitation is prohibited with the exception of compassionate care visitation.
  2. Outdoor visitation may occur in an outdoor space designated by the facility. Please contact the facility for further information.
  3. Virtual visitation is available to all residents via several video conferencing platforms.
  4. Compassionate care visitation is considered on a case-by-case basis.
  5. Beauticians/Barbers are not permitted.


PHASE 2 (Subject to the approval of the local health department):

  1. Indoor visitation may occur in indoor spaces designated by the facility. Please contact the facility for further information.
  2. Outdoor visitation may occur in an outdoor space designated by the facility. Please contact the facility for further information.
  3. Virtual visitation will continue to be available to all residents via videoconferencing platforms.
  4. Compassionate care visitation is considered on a case-by-case basis.
  5. Beauticians/Barbers are not permitted.


PHASE 3 (Subject to the approval of the local health department):

  1. Indoor visitation may occur in indoor spaces designated by the facility. Please contact the facility for further information.
  2. Weather permitting, outdoor visitation is strongly preferable to indoor visitation.
  3. Outdoor visitation permitted. See Phase 2.
  4. Virtual visitation is available to all residents via videoconferencing platforms.
  5. Compassionate care visitation is considered on a case-by-case basis.
  6. Beauticians/Barbers are permitted.
